Waiver

PLEASE READ CAREFULLY. THIS IS A RELEASE OF LIABILITY AND WAIVER OF CERTAIN LEGAL RIGHTS.

The attendee(s) and/or the parent/guardian of the attendee(s) agree and understand that swimming is a HAZARDOUS activity. The attendee recognizes that there are risks inherent in swimming, including but not limited to, paralyzing injuries and death. The attendee hereby agrees to attend Dive-In Movies and hereby agrees to indemnify and hold harmless the Hotel Zazz, HERstory Hospitality, Z Lounge, Dust Wave, Sopapilla Productions, and its owners, management, agents, representatives and employees against any liability resulting from any injury that may occur to the attendee while attending Dive-In Movies. The attendee also agrees to indemnify the Hotel Zazz, HERstory Hospitality, Z Lounge, Dust Wave, and Sopapilla Productions for any damages incurred arising from any claims, demand, action or cause of action by the attendee.

PHOTO RELEASE

I understand that photos and/or video are occasionally taken at Dive-In Movies. The Hotel Zazz, HERstory Hospitality, Z Lounge, Dust Wave, and Sopapilla Productions may use photos and/or video of myself and/or child(ren) for publicity and/or promotional purposes.
